2 저널기사 A Historical Perspective on the Development of Intraoperative Lymphatic Mapping and Selective Lymphadenectomy - Intraoperative lymphatic mapping (ILM) and selective lymphadenectomy are revolutionary concepts that, in a short period, have shown the potential to alter dramatically the management of many patients with solid neoplasms. The rapid adaptation of this approach to the staging of solid neoplasms by the surgical oncology community has resulted in an explosion of data. Initially described as a surgical technique in which each surgeon had to climb a learning curve, ILM and selective lymph node dissection (SLND) are now recognized as a multidisciplinary surgical approach to the management of the patient with cutaneous melanoma and breast cancer. The potential values of ILM and SLND are being examined vigorously now in numerous other solid neoplasms./ 미리보기

6 저널기사 Biologic and Clinical Significance of Lymphadenectomy - Interest in the lymphatic system and its relationship to metastases has developed owing to renewed interest in sentinel node biopsy. This article summarizes the anatomy, physiology, and biology of the lymphatic system and lymph node metastases, and reviews studies of lymph node metastases and surgical resection of cancers in different anatomic sites. On the basis of these studies, the authors conclude that lymph node metastasis functions as an indicator of prognosis, not the controlling or determining factor of prognosis. Thus, varying degrees of treatment of regional lymph nodes and metastases do not seem to be controlling factors in the outcome of cancer./ 미리보기

8 저널기사 Computer Databases for the Sentinel Lymph Node Program - At the University of California San Francisco Comprehensive Cancer Center, a computer database system was created using the FileMaker Pro database program. The system allows the clinical team to bring together data from all aspects of a multidisciplinary effort toward improving daily clinical management, for research, and ultimately, for the clinical standard of care for melanoma patients. With modest objectives during the initiation of the database, its adaptability has allowed it to become an integral link in all the efforts of the melanoma program. For the future, tapping into the immense potential of the Internet has begun to organize multicenter efforts. Through nationwide collaboration, the Sentinel Lymph Node Working Group will be able to follow, on a larger scale, the successes of the FileMaker pro database at the UCSF Comprehensive Cancer Center./ 미리보기

10 저널기사 CT and MR Imaging Evaluation of Neck Infections with Clinical Correlations - Infection of the neck is a common clinical problem in all age groups, especially children and young adults. The clinical symptoms and signs are often suggestive of the diagnosis. Imaging studies including CT and MR imaging are frequently required to confirm the diagnosis but more importantly to localize the infectious process and search for and delineate an abscess cavity. Ultrasound has also been used in the evaluation of superficial neck infections, especially to determine fluid accumulation. Conventional films consisting of an anteroposterior and lateral view were the examination before the introduction of CT in 1972. Conventional films can still be used for a preliminary survey, especially of the retropharyngeal space when there is a question of a retropharyngeal phlegmon or abscess./ 미리보기

16 저널기사 Future Perspectives on Selective Sentinel Lymphadenectomy - Selective sentinel lymphadenectomy is a standard staging procedure for patients with melanoma and is rapidly evolving into a standard procedure for breast cancer. A well-trained, multidisciplinary team is essential for the success of such a procedure. Clinical trials are under way to determine the clinical significance of sentinel lymph nodes in melanoma and breast cancer. The techniques of harvesting are being developed in other solid cancers, such as gynecologic and gastrointestinal cancer. 17 저널기사 Glomus Faciale, Glomus Jugulare, Glomus Tympanicum, Glomus Vagale, Carotid Body Tumors, and Simulating Lesions: Role of MR Imaging - Glomus complex tumors (paragangliomas) of the temporal bone are classified as either glomus jugulare or glomus tympanicum, depending on their sites of origin. Glomus jugulare tumors arise from the glomus bodies around the jugular bulb and along the course of either Jacobson's nerve, the tympanic branch of the glossopharyngeal nerve or Arnold's nerve, the auricular branch of the vagus nerve. Glomus tympanicum tumors arise from the glomus bodies along the course of Jacobson's nerve as it courses from the tympanic canaliculus over the promontory. Glomus faciale tumors are another rare and interesting type of paragangliomas of the temporal bone that arise from glomus bodies along the course of Arnold's nerve into the fallopian canal. 18 저널기사 Imaging of Benign and Malignant Soft Tissue Tumors of the Neck - Soft tissue tumors of the neck are a heterogeneous group of neoplasms arising from adipose, muscular, and fibrous tissue. With the exception of lymphomas, they account for only a small fraction of neck masses. Nevertheless, accurate diagnosis is important since the behavior of these neoplasms differs markedly from each other and from other head and neck masses. Noninvasive imaging, usually with CT and MR imaging, plays an important role in diagnostic evaluation and treatment planning for these tumors. In some cases, imaging features may be suggestive of a single entity. In most cases, imaging is needed to assess the location and extent of the tumor prior to biopsy or excision.
