CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - A Comparison of Fentanyl, Sufentanil, and Remifentanil for Fast-Track Cardiac Anesthesia - IMPLICATIONS: To conserve resources for cardiac surgery, fentanyl-, sufentanil-, and remifentanil-based anesthetics were compared for duration of mechanical ventilation, intensive care unit length of stay, hospital length of stay, and cost. The shorter-acting anesthetics, sufentanil and remifentanil, produced equally rapid extubation, similar stays, and similar costs to fentanyl: Thus, any of these opioids can be recommended for fast-track cardiac surgery./

Induction with sevoflurane is associated with inferior but faster conditions for intubation of the trachea. The recovery characteristics were similar in the three groups./

The decision to use one block or the other might, therefore, reasonably be influenced by the relative safety of the superficial block compared with the combined block, because previous work suggests the deep injection is associated with a more frequent complication rate/

Olprinone was a more potent pulmonary vasodilator and bronchodilator than aminophylline. In addition, olprinone may produce greater effects on phosphodiesterase-3 in pulmonary vascular than in airway smooth muscle./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - Allosteric Modification of Oxygen Delivery by Hemoglobin - IMPLICATIONS: We studied the safety and tolerance of an investigational drug, RSR13 (Allos Therapeutics, Denver, CO), in general surgery patients. This drug, which increases the amount of oxygen available to the body, was well tolerated by the 17 patients who received it. There were clinically relevant increases in serum creatinine in 3 patients, indicating a decrease in renal function, but these increases were short-lived and resolved without treatment./
