CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - CASE REPORTS - The Successful Use of Hemoglobin-Based Oxygen Carrier as a Primary Blood Substitute During Abdominal Aneurysm Repair with Large Blood Loss - IMPLICATIONS: We present the stable intraoperative course of a patient who experienced a large blood loss (8 L) during elective repair of abdominal aortic aneurysm. Intraoperatively, the patient received no allogeneic blood, but only cell saver blood (2.3 L) and ultrapurified polymerized bovine hemoglobin (0.89 L). Administration of hemoglobin-based blood substitute in selected cases may effectively reduce allogeneic blood requirements./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - CASE REPORTS - Transient Recurrent Laryngeal Nerve Palsy After Failed Placement of a Transesophageal Echocardiographic Probe in an Anesthetized Patient - IMPLICATIONS: This case report shows that transient recurrent laryngeal nerve palsy can be caused by unsuccessful attempts to place a transesophageal echocardiographic probe./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - CASE REPORT - Sudden Respiratory Arrest Resulting From Brainstem Embolism in a Patient Undergoing Endovascular Abdominal Aortic Aneurysm Repair - IMPLICATIONS: Surgery on the aorta is a great challenge for the anesthesiologist, even with newly developed and less invasive stent-graft procedures. The case of a fatal cerebral embolism during endovascular repair of an abdominal aortic aneurysm is reported, and the potential mechanisms underlying this unexpected complication are discussed./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - CASE REPORT - The Safety of Intraoperative Transesophageal Echocardiography: A Case Series of 7200 Cardiac Surgical Patients - IMPLICATIONS: The overall morbidity (0.2%) and mortality (0%) rates of intraoperative transesophageal echocardiography (TEE) were determined in a retrospective case series of 7200 adult, anesthetized cardiac surgical patients. The most common source of TEE-associated morbidity was odynophagia (0.1%), which resolved with conservative management. These results suggest that TEE is a safe diagnostic tool for the management of cardiac surgical patients./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - CASE REPORT - The Use of Lepirudin for Anticoagulation in Patients with Heparin-Induced Thrombocytopenia During Major Vascular Surgery - IMPLICATIONS: The method of anticoagulation in patients undergoing major vascular surgery with a history of heparin-induced thrombocytopenia (HIT) is controversial. We present two cases in which a bolus only technique using recombinant hirudin (Lepirudin or Refludan) was used successfully in patients with HIT scheduled for vascular surgery./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - Cibenzoline Has an Inhibitory Effect on Vasorelaxation Mediated by Adenosine Triphosphate-Sensitive K+ Channels in the Rat Carotid Artery - IMPLICATIONS: In isolated rat carotid artery, cibenzoline (10-6 to 10-5 M) reduced vasorelaxation to levcromakalim in a concentration-dependent fashion. These results suggest that from the clinically relevant concentrations, a novel class la antiarrhythmic drug, cibenzoline, impairs carotid vasodilation mediated by adenosine triphosphate-sensitive K+ channels./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - Clonidine-Induced Vasoconstriction in Awake Volunteers - IMPLICATIONS: IV clonidine, at doses that decrease blood pressure, causes arterial vasoconstriction in awake subjects. These data suggest that an a-2 agonist with a high a-2a/a-2b selectivity should provide more profound sedative and analgesic effects with less undesirable vasoconstrictive effects./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - Cognition After Coronary Artery Surgery Is Not Related to Postoperative Jugular Bulb Oxyhemoglobin Desaturation - IMPLICATIONS: Postoperative cerebral hypoperfusion, indicated by oxyhemoglobin desaturation < 50%, occurs in many patients after coronary artery bypass graft surgery. However, this measurement of hypoperfusion does not appear to influence long-term cognition after coronary artery bypass graft surgery./

CARDIOVASCULAR ANESTHESIA - (Society of Cardiovascular Anesthesiologists) - Comparing Doppler Ultrasonography and Cerebral Oximetry as Indicators for Shunting in Carotid Endarterectomy - IMPLICATIONS: Although cerebral oximetry was easy to apply but considerably unspecific (13% false positives), transcranial Doppler ultrasonography was more accurate in indicating the risk of cerebral hypoperfusion during carotid cross-clamping. Additionally, the improvement in cerebral blood flow velocity after inducing arterial hypertension might prevent cerebral hypoperfusion during cross-clamping./
