AMBULATORY ANESTHESIA - (Society for Ambulatory Anesthesia) - Perioperative Myocardial Ischemia in Cataract Surgery Patients: General Versus Local Anesthesia - IMPLICATIONS: In patients with risk factors for ischemic heart disease, cataract surgery occurs in 30% of the cases with ischemic events of short duration. There were significantly less intraoperative episodes in the local anesthesia group, suggesting that local anesthesia may be safer than general anesthesia in patients during this type of surgery./

AMBULATORY ANESTHESIA - Society for Ambulatory Anesthesia - SPECIAL ARTICLE - The Good, the Bad, and the Ugly: Should We Completely Banish Human Albumin from Our Intensive Care Units? - IMPLICATIONS: Human albumin is still widely used in critically ill patients for volume replacement therapy or for correcting hypoproteinemia. Most meta-analyses on the value of albumin administration are over 15 yr old and raise more questions than they answer. With the help of a MEDLINE analysis, we examined more recent studies in humans using albumin. Most of these studies have recommended a very cautious use of albumin in critically ill patients/

AMBULATORY ANESTHESIA (Society for Ambulatory Anesthesia) - The Effect of Bispectral Index Monitoring on End-Tidal Gas Concentration and Recovery Duration After Outpatient Anesthesia - IMPLICATIONS: Adoption of Bispectral index monitoring throughout an entire operating room was associated with use of lesser concentrations of sevoflurane to maintain anesthesia and reduced recovery duration in men undergoing general anesthesia for ambulatory surgery./

AMBULATORY ANESTHESIA - (Society for Ambulatory Anesthesia) - The Effects of Small-Dose Ketamine on Propofol Sedation: Respiration, Postoperative Mood, Perception, Cognition, and Pain - IMPLICATIONS: Simultaneous use of small-dose ketamine during propofol sedation improves ventilation and produces positive mood states in the recovery room. The combination of drugs may also provide earlier return of cognitive function./

AMBULATORY ANESTHESIA - (Society for Ambulatory Anesthesia) - The Lack of Systematic Month-to-Month Variation Over One-Year Periods in Ambulatory Surgery Caseload-Application to Anesthesia Staffing - IMPLICATIONS: The average number of ambulatory surgery cases performed with an anesthesia provider each day in the United States per 10,000 population did not vary systematically month to month on a medium-range (11-mo) basis. These findings suggest that the relatively simple statistical methods that are available to estimate future anesthesia workload will work for many anesthesia groups./

AMBULATORY ANESTHESIA - (Society for Ambulatory Anesthesia) - The Use of Dexamethasone for Preventing Postoperative Nausea and Vomiting in Females Undergoing Thyroidectomy: A Dose-Ranging Study - IMPLICATIONS: We evaluated four doses of IV dexamethasone in preventing postoperative nausea and vomiting in women undergoing thyroidectomy. We found that dexamethasone 5 mg was as effective as dexamethasone 10 mg, and was more effective than saline control. Dexamethasone 5 mg is suggested to be the minimum effective dose for this purpose./

AMBULATORY ANESTHESIA - (Society of Ambulatory Anesthesia) - Postoperative Analgesia at Home After Ambulatory Hand Surgery: A Controlled Comparison of Tramadol, Metamizol, and Paracetamol - IMPLICATIONS: Despite pain medication, up to 40% of patients experienced pain at home after ambulatory hand surgery. Of the three analgesic drugs evaluated in this double-blinded study, tramadol was the most effective but was also associated with most patient dissatisfaction because of an increased incidence of nausea and dizziness. Metamizol provided somewhat better analgesia than acetaminophen. There is a need for better oral pain medication at home./
