ADVANCE IN SURGICAL TECHNIQUE - The Use of Fibrin Sealant for Prosthetic Mesh Fixation in Laparoscopic Extraperitoneal Inguinal Hernia Repair - Laparoscopic hernia repair involves the fixation of the prosthetic mesh in the preperitoneal space with staples to avoid displacement leading to recurrence. The use of staples is associated with a small but significant number of complications, mainly nerve injury and hematomas./

ADVANCES IN SURGICAL TECHNIQUE - Early Experience With Laparoscopic Approach for Solid Liver Tumors: Initial 16 Cases - Fifteen patients underwent 16 consecutive laparoscopic liver resections for solid tumors with no operative mortality and low morbidity. Laparoscopic surgery appears to be a safe procedure for benign tumors in selected cases. Laparoscopic management of malignant lesions remains controversial./

ADVANCES IN SURGICAL TECHNIQUE - Liver Resection Without Total Vascular Exclusion: Hazardous or Beneficial? An Analysis of Our Experience - Total vascular exclusion (TVE) is recommended for safe liver surgery, at least in the case of resection of the paracaval portion of the liver. However, it has some drawbacks due to its invasiveness. The authors report a retrospective analysis in terms of safety and radicality of liver resection performed without TVE. No mortality, low morbidity, low rate of blood transfusions, and complete tumor clearance in all patients were experienced./

ADVANCES IN SURGICAL TECHNIQUE - Surgical Technique for Right Lobe Adult Living Donor Liver Transplantation Without Venovenous Bypass or Portocaval Shunting and With Duct-to-Duct Billary Reconstruction - The authors describe the technique for adult living donor liver transplantation without veno-venous bypass, caval clamping, or porto-caval decompression. In addition, modifications aimed at preserving the blood supply to the bile duct are described that may allow a direct duct-to-duct biliary reconstruction in selected cases./

ADVANCES IN SURGICAL TECHNIQUE - Sutureless Closed Hemorrhoidectomy: A New Technique - A new technique of radical hemorrhoidectomy using an electrothermal device originally devised to seal vessels in abdominal operations was compared with conventional Milligan-Morgan operation. The operative time and time off work were significantly shorter in the study group (t-test, P < .001). There were also less postoperative complications in this group./

Analysis of Surgical Success in Preventing Recurrent Acute Exacerbations in Chronic Pancreatitis - Chronic pancreatitis requires operative intervention to treat chronic abdominal pain. The authors have evaluated the subset of patients with chronic pancreatitis whose disease is manifested by intermittent acute exacerbations. The data confirm that recurrent acute attacks may be successfully prevented or ameliorated by operation./
