10 저널기사 Antibiotic Resistance - Antibiotic resistance is agent specific, and is not related to volume or duration of use nor limited to certain antibiotic classes. Antibiotics should be viewed as having little or no resistance potential or having a high resistance potential. Antibiotics with a low resistance potential should not be restricted but those with a high resistance potential should be restricted. The only effective mechanism to control antibiotic resistance is by limiting hospital formularies to include only antibiotics with little or no resistance potential. Physicians in the ambulatory setting need to be more selective in antibiotic prescribing and preferentially select antibiotics with a low resistance potential./ 미리보기

12 저널기사 Antienterococcal Antibiotics - The spread of enterococci as a major cause of nosocomial infections and the increasing resistance of these organisms to all currently available antibiotics, including glycopeptides, has rendered therapy of these infections particularly difficult. Mechanisms of resistance to antimicrobial agents are now understood to a significant extent. Therapeutic regimens are often based on the results of experimental studies but most often have not been validated by large clinical trials. 20 저널기사 Clinical use of Fluoroquinolones - The quinolones have undergone a "structural evolution" since the inadvertent discovery of nalidixic acid in 1962. A better understanding of structure-activity and structure-toxicity relationships has allowed chemists to modify the adaptable quinolone structure to enhance or limit the extent of antimicrobial activity and to improve various other product attributes such as the pharmacokinetic profile, tolerability, drug interaction potential, and toxicity of each new agent. Ciprofloxacin and levofloxin/oflofloxin have demonstrated relatively clean safety profiles over several years of clinical use. Newer fluorquinolones should be fully evaluated by clinicians prior to their use so as to avoid potentially life-threatening adverse events. Several newer quinolones are available or are being investigated for clinical use, including gatifloxacin, moxifloxican, clinafloxacin, and gemifloxacin.
