Laparoscopic Bariatric Surgery - At present, laparoscopic bariatric surgery is a controversial topic among bariatric and laparoscopic surgeons. Although difficult to perform, the traditional procedures used for treating severe obesity are now being performed successfully using a laparoscopic approach. In addition, a new procedure, adjustable gastric banding, has been used in Europe; however, it lacks US Food and Drug Administration approval and adequate scientific follow-up data about long-term effectiveness. Appropriate patient selection and adherence to the principles that have been learned by experience through open bariatric surgery remain of paramount importance for the success of laparoscopic bariatric operations/

Laparoscopic Colectomy - A laparoscopic approach to colon resection has yet to achieve routine acceptance. Improved instrumentation and studies suggesting results that are equivalent to traditional colectomy in cancer surgery now make a laparoscopic approach more attractive than it previously was. This article presents various techniques of colon resection and the animal and human studies relevant to immune function and cancer surgery/

Laparoscopic Colectomy in Diverticular and Crohn's Disease - Laparoscopic treatment of benign conditions of the colon, rectum, and anus are well accepted. Three potentially advantageous procedures include sigmoid colectomy, ilieocolic resection, and stoma creation for sigmoid diverticulitis, terminal ileal Crohn's disease, and severe perianal Crohn's disease. Other technically feasible procedures include laparoscopically assisted total abdominal colectomy for colonic Crohn's disease, laparoscopic total proctocolectomy for colonic and anorectal Crohn's disease, and laparoscopic secondary ileoproctostomy or coloproctostomy, such as Hartmann's reversal procedures. With these various procedures, significant benefits can be expected relative to the length of ileus, the possibility of adhesion formation, and the patient's experience with pain, period of hospitalization, cosmesis, and disability/

Laparoscopic Esophageal Surgery - In its 9-year history, laparoscopic esophageal surgery has become second only to gallbladder surgery in the frequency of minimally invasive procedures performed in routine surgical practice. Laparoscopic fundoplication has assumed a central role in the surgical treatment of gastroesophageal reflux. Laparoscopic myotomy has emerged as the optimal form of therapy for achalasia, and staging laparoscopy has been identified as an important adjunct to the preoperative evaluation of esophageal and gastroesophageal junction carcinoma. Laparoscopic paraesophageal hernia repair and remedial laparoscopic antireflux surgery currently are gaining acceptance. Laparoscopic gastroplasty, esophagectomy, and diverticulectomy are undergoing clinical trials, and their roles remain to be defined/

Laparoscopic Gastric Resection - This article focuses on laparoscopic gastric resections for neoplastic disease, gastric cancer, and gastric mesenchymal tumors. Established oncologic principles governing resections for neoplastic disease must not be overlooked in the laparoscopic approach to surgical management. Evidence-based information on surgical management of gastric cancer and stromal stomach tumors is presented, and the laparoscopic procedures that can be undertaken without compromising the clinical outcome are surveyed/

Laparoscopic Hernia Repairs: The Importance of Cost as an Outcome Measurement at the Century's End - Laparoscopic hernia repair has generated controversy since its introduction in the early 1990s. Despite clinical studies showing low hernia recurrence rates, rapid recovery rates, and high patient preference, laparoscopic repairs are not widely performed by surgeons -- Owing mainly to the difficult learning curve and the reportedly high costs of the procedure. Cost effectiveness and quality of life analyses further support the use of laparoscopic repairs in many cases. The establishment of cost-driven practice guidelines can reduce the high cost of laparoscopy/

Laparoscopic Management of Benign Liver Disease - Laparoscopic liver surgery can be accomplished safely in selected patients with benign liver lesions, provided that appropriate laparoscopic skills and technology are available. The size of the lesions is less important than their anatomic locations in anterolateral regions. Small, benign, solid tumors located in anterior liver segments and giant solitary cysts can be managed safely and effectively using the laparoscopic approach. The laparoscopic management of polycystic liver disease should be reserved for patients with a limited number of large, anteriorly located cysts/

Laparoscopic Sonography of the Biliary Tree and Pancreas - Laparoscopic sonography is an important tool for pancreaticobiliary surgeons. It is used to evaluate and manage common bile duct stones, pancreatic neoplasia, pseudocytes, and tumors of the biliary tree. This article discusses the rationale for using laparoscopic sonography in the clinical setting, reviews the techniques, findings, and experience of the authors and other investigators, and presents advantages and disadvantages/

Laparoscopic Splenectomy - Laparoscopic splenectomy has become the procedure of choice for various hematologic disorders. It carries less significant morbidity than open surgery, has comparable hematologic efficacy, and offers the unique advantages of the minimally invasive approach. To be performed and taught safely, however, techniques must be applied meticulously, including minimizing operative bleeding, improving hematologic response by searching carefully for accessory spleens, and reducing the risk of splenosis secondary to capsular fractures. With data accumulating, evidence is increasing that laparoscopic splenectomy is the gold standard when surgery is indicated for selected hematologic diseases/
