LABORATORY INVESTIGATIONS - Potency of Bupivacaine Stereoisomers Tested In Vitro and In Vivo: Biochemical, Electrophysiological, and Neurobehavioral Studies - Although the stereopotency ratio of bupivacaine enantiomers tested in vitro by electrophysiological or biochemical methods can reach values of R:S = 3:1, representing the stereoselectivity of binding to inactivated Na+ channels, functional deficits from sciatic nerve block in vivo at clinical concentrations show stereoselectivities of only 1.2-1.3 at the most, a finding consistent with (1) the weak stereopotency for blocking resting Na+ channels, which may be primarily involved in impulse blockade in vivo; (2) the different potency ratios of the enantiomers for inhibiting Na+ versus K+ channels involved in impulse propagation; or (3) the differential effects of the enantiomers on the local vasculature and thus on their own local removal/
