SCIENTIFIC PAPERS - Complex Abdominal Wall Reconstruction: A Comparison of Flap and Mesh Closure - Patients undergoing repair of 106 complex abdominal defects (n = 104) were analyzed to compare results of mesh and flaps and define a treatment protocol for abdominal wall reconstruction. Criteria used to predict complexity in abdominal wall repair were identified. Both mesh and flap techniques, when appropriately selected, are safe and effective in the restoration of abdominal wall continuity./

SCIENTIFIC PAPERS - Determinants of Skeletal Muscle Catabolism After Severe Burn - The authors used stable isotope methodology to quantify leg skeletal muscle protein catabolism in 123 burned patients, and found admission weight, bum size, time to wound excision, resting energy expenditure, and the presence or absence of sepsis to be predictive of the magnitude of the catabolic response./

SCIENTIFIC PAPERS - Does Prophylactic Octreotide Decrease the Rates of Pancreatic Fistula and Other Complications After Pancreaticoduodenectomy? Results of a Prospective Randomized Placebo-Controlled Trial - This prospective, randomized, double-blinded, placebo-controlled single-institution trial evaluated the role of prophylactic octreotide in altering morbidity and mortality after pancreaticoduodenectomy. The trial evaluated 211 patients, half of whom received octreotide and the other half a saline placebo. The postoperative morbidity, mortality, and length of hospital stay were similar between the groups. These data indicate that prophylactic octreotide does not reduce the incidence of complications after pancreaticoduodenectomy/

SCIENTIFIC PAPERS - Endovascular Grafts and Other Image-Guided Catheter-Based Adjuncts to Improve the Treatment of Ruptured Aortoiliac Aneurysms - The feasibility, safety, and effectiveness of a new management approach for ruptured aortoiliac aneurysms is reported. This approach features the use of a fluoroscopically guided transbrachial supraceliac occlusion balloon and, whenever possible, an endovascular graft with intraoperative customization capability; 20 patients were treated with an endovascular graft, five underwent open repair, and 22 survived./

SCIENTIFIC PAPERS - Endpoints of Resuscitation of Critically Injured Patients: Normal or Supranormal? A Prospective Randomized Trial - Severely injured patients randomized to receive standard resuscitation had identical survival with patients randomized to receive early optimization; 30% of patients randomized to optimization did not achieve optimal hemodynamic values, whereas 40% of patients randomized to standard resuscitation reached such values spontaneously. Patients from both groups who reached optimal values had higher survival rates than patients who did not/

SCIENTIFIC PAPERS - Gallbladder Cancer: Comparison of Patients Presenting Initially for Definitive Operation With Those Presenting After Prior Noncurative Intervention - This study reports the results of treatment for 410 patients with gallbladder cancer over a 13-year period at a single referral center. Resectability, extent of liver involvement, and presence of nodal metastases were the most important determinants of outcome. Presentation after prior noncurative surgical exploration did not preclude radical resection or long-term survival./

SCIENTIFIC PAPERS - Gastrointestinal Carcinoids: Characterization by Site of Origin and Hormone Production - A series of 336 gastrointestinal carcinoid patients is described in relation to presenting symptoms, neurohormone levels, stage at presentation, and survival. Although pancreatic and midgut carcinoids have worse prognoses overall, stage-corrected survival reveals improved survival with midgut carcinoids./

SCIENTIFIC PAPERS - Gut-Derived Sepsis Occurs When the Right Pathogen With the Right Virulence Genes Meets the Right Host: Evidence for In Vivo Virulence Expression in Pseudomonas aeruginosa - Pseudomonas aeruginosa can cause a state of gut-derived sepsis in mice due to presence of a key virulence factor, the PA-I lectin/adhesin. This study demonstrates that the PA-I lectin/adhesin is "in vivo expressed" in the intestinal tract of mice by the counteregulatory response to surgical stress./

SCIENTIFIC PAPERS - Health-Related Quality of Life After Different Types of Solid Organ Transplantation - Karnofsky Performance Status, SF-36, and Psychosocial Adjustment to Illness Scale were obtained for 100 liver, 94 heart, 112 kidney, and 65 lung transplant patients. Different types of transplant patients had differing results before transplantation. All measures improved after transplantation, but the trajectories were different for each type of transplant patient. Path analysis indicated that the presence of diabetes and time after transplantation predicted Karnofsky score. The most recent Karnofsky score and education level predict subjective health-related quality of life after solid organ transplantation./

SCIENTIFIC PAPERS - Ileal Pouch Anal Anastomosis Without Ileal Diversion - The authors present a large series of "one-stage" triple-stapled ileoanal pouch procedures with a 12% anastomotic leak rate in which only one third (4% of total) required emergent ileostomy diversion. No significant differences were noted in leak rate with regard to sex, obesity, age, or patient use of steroids, and long-term fecal function was excellent./

SCIENTIFIC PAPERS - Impact of Current Management Practices on Early and Late Death in More Than 500 Consecutive Cardiac Transplant Recipients - The authors demonstrate that not only does cardiac transplantation provide satisfactory long-term survival for patients with end-stage heart disease, but also previously identified risk factors are no longer significant in adversely affecting posttransplant survival. Steady improvement in survival during this period argues that advances in transplantation have offset the increasing acuity of transplant recipients/

SCIENTIFIC PAPERS - Increased Rates of Donation With Laparoscopic Donor Nephrectomy - This study examined the effects of laparoscopic nephrectomy and a recipient education program on the identification of living kidney donors. Introduction of these program changes increased the proportion of new kidney transplant candidates who were able to receive a live donor kidney transplant from 12.2% to 24.7%/

SCIENTIFIC PAPERS - Laparoscopic Repair of Giant Paraesophageal Hernia: 100 Consecutive Cases - Over a 4-year period, 100 patients underwent laparoscopic repair of a giant paraesophageal hernia (PEH). Follow-up included heartburn scores and quality-of-life measurements. Laparoscopic repair of giant PEH was successfully accomplished in 97% of patients with no 30-day mortalities, one delayed operative mortality from the complications of a perioperative stroke, acceptable morbidity, 2-day length of stay, and good intermediate results./

SCIENTIFIC PAPERS - Long-Term Survival After Liver Transplantation in 4,000 Consecutive Patients at a Single Center - Little is known about causes of late graft failure and death in liver transplant patients. The authors compared the impact of factors such as diagnosis, age, sex, and time period of transplantation to the causes of death and graft loss in 4,000 liver transplant recipients over a 20-year period at a single center. Long-term survival was significantly better in the pediatric age group, in female recipients, and in patients transplanted after 1990. Rates of retransplantation for acute or chronic rejection were significantly lower under tacrolimus-based immunosuppression./

SCIENTIFIC PAPERS - Multifactorial Analysis of Surgical Outcome in Patients with Unruptured Middle Cerebral Artery Aneurysms - Patients with unruptured middle cerebral artery aneurysms were analysed for the presence of the following factors: Aneurysm size, broad neck, intraaneurysmal plaque, clipping of multiple aneurysms, temporary occlusion, clip repositionings, and the use of multiple clips. Each patient was analyzed for the Factor Accumulation Index (FAI). The authors believe that it is possible to predict outcome by calculating FAI. Postoperative morbidity increases with FAI within a range of 3 to 4./

SCIENTIFIC PAPERS - Outcomes After Laparoscopic Roux-en-Y Gastric Bypass for Morbid Obesity - Laparoscopic Roux-en-Y gastric bypass is feasible and safe, resulting in reduced perioperative morbidity and recovery time. Operative complication rates appear to be comparable to that of the open approach. Excess body weight loss at 24 and 30 months was 83% and 77%, respectively, resulting in a significant improvement in comorbidities and quality of life./

SCIENTIFIC PAPERS - Postoperative Bile Duct Strictures: Management and Outcome in the 1990s - A single-institution experience in the surgical management of 156 patients with postoperative bile duct strictures and/or major bile duct injuries during the 1990s is reported. Routine management consisted of Roux-en-Y hepaticojejunostomy constructed over one or more transhepatic silastic stents. A successful outcome, without the need for invasive diagnostic or therapeutic procedures, was obtained in 91% of patients, with a mean follow-up of 57.5 months. Overall success, including patients undergoing secondary treatment for restricturing, was 98%/
