Sedation-Analgesia in the Pediatric Intensive Care Unit - Children in intensive care units commonly require sedation and analgesia, both for the management of procedure-associated pain and fear, and for ongoing analgesic and antianxiolytic needs. This article discusses management strategies for providing pain relief and sedation for critical care patients. The use of indication-based decision making for choosing therapeutic options is described, and safety, monitoring, and organizational issues are discussed. The use of alternatives to systemic agents is presented, including local and regional anesthesia./

Septic Shock - Warwick Butt - Infection is problematic because it affects many patients (adults and children), is a major cause of death in intensive care units (ICU) worldwide, and uses a large amount of hospital resources. The mortality rate among patients with septic shock varies but approximates 40% in infected patients admitted to ICUs. Because of the large number of adults dying of sepsis, many resources are expended. Children are physiologically different from adults, but nonetheless, many similarities exist with respect to the response to septic shock./

Shoulder Rehabilitation Strategies, Guidelines, and Practice - Shoulder rehabilitation can best be understood and implemented as the practical application of biomechanical and muscle activation guidelines to the repaired anatomic structures in order to allow the most complete return to function. The shoulder works as a link in the kinetic chain of joint motions and muscle activations to produce optimum athletic function. Functional shoulder rehabilitation should start with the establishment of a stable base of support and muscle facilitation in the truck and legs, and then proceeds to the scapula as healing is achieved and proximal control is gained. The pace of this "flow" of exercises is determined by achievement of the functional goals of each segment in the kinetic chain. In the early rehabilitation stages, the incompletely healed shoulder structures are protected by exercises that are directed towards the proximal segments.
