저널기사 TECHNICAL COMMUNICATION - A Comparison of Atenolol, Labetalol, Esmolol, and Landiolol for Altering Human Neutrophil Functions - IMPLICATIONS: Neutrophils play a pivotal role in the antibacterial host defense system. Atenolol, labetalol, esmolol, and landiolol at clinically relevant concentrations failed to change neutrophil functions. Ourfindings indicate that we may be able to use these b-antagonists without great caution in clinical settings./ 미리보기

저널기사 TECHNICAL COMMUNICATION - A Study of the Paravertebral Anatomy for Ultrasound-Guided Posterior Lumbar Plexus Block - IMPLICATIONS: We investigated the feasibility of posterior paravertebral sonography as a basis for ultrasound-guided posterior lumbar plexus blockades. Posterior paravertebral sonography proved to be a reliable as well as accurate imaging procedure for visualization of the lumbar paravertebral region except the lumbar plexus./ 미리보기

저널기사 TECHNICAL COMMUNICATION - Designing Meaningful Industry Metrics for Clinical Productivity for Anesthesiology Departments - IMPLICATIONS: Clinical productivity measurements that account for differences in clinical settings and concurrencies provided more precise comparisons between two anesthesiology groups. The data show that different concurrencies confound the current industry standard, "per full-time equivalent" measurements, whereas "per operating room site" and "per case" measurements allowed for more meaningful comparisons./ 미리보기

저널기사 TECHNICAL COMMUNICATION - Intraoperative Monitoring of the Recurrent Laryngeal Nerve in 151 Consecutive Patients Undergoing Thyroid Surgery - IMPLICATIONS: We present a technique of intraoperative monitoring of the recurrent laryngeal nerve using a surface electrode attached to a routine endotracheal tube. The technique proved noninvasive, easy to use, and reliable in 151 prospective consecutive patients for preventing permanent laryngeal nerve damage in thyroid surgery./ 미리보기

저널기사 TECHNICAL COMMUNICATION - Practical Issues in Bispectral Analysis of Electroencephalographic Signals - IMPLICATIONS: The aim of this report was to confirm the methodology of bispectral analysis of electroencephalogram. In developing a software for real-time bispectral analysis, we encountered several practical problems in bispectrum calculation. We settled those and concluded that 3 min of monitoring are required to obtain reliable and reproducible bicoherence values./ 미리보기

저널기사 TECHNICAL COMMUNICATION - Productivity Versus Availability as a Measure of Faculty Clinical Responsibility - IMPLICATIONS: Accountability of clinical activities by faculty is crucial to the financial status of any department of anesthesia. We hypothesized that methods of availability (e.g., amount of time scheduled for clinical activities) versus productivity measure (actual amount of clinical care delivered) would be quite different between faculty and differing subspecialty groups. Even though the availability system distributed clinical time on an equal basis, there was a wide difference of clinical productivity within and between specialty groups. We conclude that a productivity measure (i.e., billable hours) is a more accurate reflection of faculty productivity than an availability system and is more in line with departmental sources of financial income./ 미리보기

저널기사 TECHNICAL COMMUNICATIONS - A Simple and Inexpensive Nasal Cannula to Prevent Rebreathing for Spontaneously Breathing Patients Under Surgical Drapes - IMPLICATIONS: A new nasal cannula that provides oxygenation and suctioning simultaneously prevents rebreathing during surgery in spontaneously breathing patients under surgical drapes. When air is not suctioned, inspired CO2 levels increase significantly, whereas suctioning prevents this increase. Expiratory CO2, respiratory rate, heart rate, and arterial blood pressure remain stable regardless of suctioning./ 미리보기

저널기사 TECHNICAL COMMUNICATIONS - Fiberoptically-Guided Insertion of Transtracheal Catheters - IMPLICATIONS: Regular use of the transtracheal catheter (TTC) both offers an opportunity for training for the difficult airway and facilitates elective endoscopic surgery. Fiberoptic guidance and exploratory puncture improve the insertion of the TTC./ 미리보기

저널기사 TECHNICAL COMMUNICATION - The Analgesic Efficacy of Patient-Controlled Bupivacaine Wound Instillation After Total Abdominal Hysterectomy with Bilateral Salpingo-Oophorectomy - IMPLICATIONS: Bupivacaine instillation via an electronic patient-controlled analgesia device provides effective analgesia after total abdominal hysterectomy with bilateral salpingo-oophorectomy./ 미리보기

저널기사 TECHNICAL COMMUNICATION - The Assessment of the Proximal Left Pulmonary Artery by Transesophageal Echocardiography and Computed Tomography in Neonates and Infants: A Case Series - IMPLICATIONS: Transesophageal echocardiography (TEE) is often used during surgical repair of congenital heart disease. In our case series of 256 newborns and infants, we found that a left paracarinal view of TEE could visualize the proximal left pulmonary artery, a frequent blind spot for TEE, in most patients, except in a few cases with anatomic variations of the esophagus in the right lateral to the vertebra./ 미리보기

저널기사 TECHNICAL COMMUNICATION - The SiBITM Connector. A New Medical Device to Facilitate Preoxygenation and Reduce Waste Anesthetic Gases During Inhaled Induction with Sevoflurane - IMPLICATIONS: The SIBITM connector is a new medical device used for vital capacity inhaled induction with sevoflurane. It allows efficient preoxygenation of patients and reduces waste anesthetic gases in the operation room during induction./ 미리보기

저널기사 TECHNOLOGY, COMPUTING, AND SIMULATION - (Society for Technology in Anesthesia) - A New, Safety-Oriented, Integrated Drug Administration and Automated Anesthesia Record System - IMPLICATIONS: Traditional methods of drug administration and record keeping in anesthesia are relatively error prone. By using sound principles of systems design and human factors psychology, we have designed and deployed a system with the aim of improving patient safety by facilitating correct drug administration and accurate anesthesia record making./ 미리보기

저널기사 TECHNOLOGY, COMPUTING, AND SIMULATION - (Society for Technology in Anesthesia) - Canister Tip Orientation and Residual Volume Have Significant Impact on the Dose of Benzocaine Delivered by Hurricaine(R) Spray - IMPLICATIONS: There is substantial variability in dose of benzocaine administered by nonmetered Hurricaine(R) spray, Canister orientation and residual volume of anesthetic significantly affect output of benzocaine and are clinically uncontrollable. Methemoglobinemia may occur even after the manufacturer's suggested spraying guidelines. We saw no correlation between dose and spraying time or number of sprays./ 미리보기

저널기사 TECHNOLOGY, COMPUTING, AND SIMULATION - (Society for Technology in Anesthesia) - CASE REPORT - Statistical Analysis by Monte-Carlo Simulation of the Impact of Administrative and Medical Delays in Discharge from the Postanesthesia Care Unit on Total Patient Care Hours - IMPLICATIONS: We review in this case study how we used computer simulation to estimate for a postanesthesia care unit (PACU) the impact on total length of stay of administrative delays from hospital wards not being available to receive patients in a timely manner from the PACU. The methodology can be applied to other delays in discharge, be they administrative or medical./ 미리보기
