Technique - Effect of Isoflurane Versus Nicardipine on Blood Flow of Lumbar Paraspinal Muscles During Controlled Hypotension for Spinal Surgery - The effects of isoflurane and nicardipine on lumbar paraspinal muscle blood flow were compared using laser Doppler flowmetry. The results showed that isoflurane will decrease, while nicardipine will increase the lumbar paraspinal muscle blood flow when they were used to achieve controlled hypotension during lumbar spinal surgery./

Technique for Repair of the Subscapularis Tendon - Subscapularis tendon injuries, if left undiagnosed, can result in significant disability for individual patients. This article outlines the anatomy, techniques of diagnosis, and a method of repair that has been successful in the senior author's hands. Early recognition and aggressive operative repair are emphasized./

Technique - Management of Fixed Sagittal Plane Deformity: Results of the Transpedicular Wedge Resection Osteotomy - A total of 13 consecutive patients with fixed sagittal deformity were corrected with the transpedicular wedge resection osteotomy. The technique is useful for the management of sagittal deformity of etiologies including iatrogenic flat back, ankylosing spondylitis, and infectious and idiopathic deformity. Coronal plane correction can also be achieved in patients with preoperative coronal imbalance./

Technique - New Method for Detecting Pedicular Wall Perforation During Pedicle Screw Insertion - The authors describe their observation of fatty particles during drilling for the pedicle screw. By assessing sensitivity and specificity and positive and negative predictive values, they conclude that this observation can be used alone or in combination with other methods to improve the accuracy of pedicle screw insertion./

Technique - On the Incidence, Cause, and Prevention of Recurrent Laryngeal Nerve Palsies During Anterior Cervical Spine Surgery - Results of an analysis of 900 cases of patients who underwent anterior cervical spine surgery showed a 6.8% incidence of recurrent laryngeal nerve injury in the first 250 cases. Deflating the endotracheal cuff and then reinflating it allows the ET to recenter in the larynx and reduced the incidence to 2%. Cadaveric studies confirm this recentering and relief of pressure on the laryngeal wall./

Technique - Postoperative Pain Control Using Epidural Catheters After Anterior Spinal Fusion for Adolescent Scoliosis - An effective epidural postoperative analgesic regimen after anterior spinal fusion is presented. Postoperative pain scores demonstrated adequate pain control. Epidural catheters can be used safely and effectively for postoperative pain control after anterior instrumentation and spinal fusion for adolescent scoliosis./

Technique - Retroperitoneal Endoscopically Assisted Minilaparotomy for Anterior Lumbar Interbody Fusion: Technical Feasibility and Complications - A new, less-invasive endoscopically assisted retroperitoneal approach for anterior lumbar interbody fusion is presented. There were no peri- or postoperative complications directly related to this novel technique. This approach appears to be a reasonable alternative to existing less invasive approaches./
